Overview

proinsulin-peptide C19-A3-pulsed tolerogenic dendritic cells (PIpepTolDC) is an autologous cell-based immunotherapy developed by Leids Universitair Medisch Centrum (LUMC) and City of Hope National Medical Center for the treatment of type 1 diabetes mellitus (T1D). The therapy consists of a patient's own (autologous) tolerogenic dendritic cells (tolDCs), derived from CD14+ monocytes collected via leukapheresis, which are matured ex vivo (typically using vitamin D3 and dexamethasone) and loaded ('pulsed') with the naturally derived proinsulin peptide C19-A3. PIpepTolDC acts as an 'inverse vaccine' designed to restore immune tolerance by presenting the C19-A3 peptide to suppress proinflammatory, autoreactive T-cell responses and induce antigen-specific regulatory T cells (Tregs), thereby halting the autoimmune destruction of insulin-producing pancreatic beta cells. It has been evaluated in Phase 1 clinical trials (such as the D-Sense trial) demonstrating safety, feasibility, and a durable reduction in autoimmune responses.
