Proligliptin + metformin is a fixed-dose combination (FDC) therapy developed by CSPC Pharmaceutical Group for the management of type 2 diabetes mellitus. Proligliptin (DBPR108) is a potent and selective inhibitor of dipeptidyl peptidase-4 (DPP-4), an enzyme responsible for the degradation of incretin hormones such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). By inhibiting DPP-4, proligliptin enhances glucose-dependent insulin secretion and suppresses glucagon release. Metformin, a biguanide, complements this action by activating AMP-activated protein kinase (AMPK) and inhibiting mitochondrial complex I, leading to reduced hepatic gluconeogenesis and improved insulin sensitivity in peripheral tissues. This combination aims to provide superior glycemic control compared to monotherapy while simplifying the treatment regimen for patients.
