Overview

Promestriene is a synthetic estrogen and a derivative of estradiol used primarily as a topical agent in the treatment of vulvovaginal atrophy and related symptoms in women. It is formulated as a 1% cream for local application. Structurally identified as the 3-propyl and 17β-methyl diether of estradiol (C22H32O2), promestriene acts locally with minimal systemic absorption and negligible systemic estrogenic effects. Its main clinical uses include alleviating symptoms associated with vaginal atrophy (such as dryness or irritation), promoting wound healing after vulvovaginal surgery or trauma (including postpartum recovery), and treating conditions like vaginitis atrophic pelvic organ prolapse and endometrial hyperplasia. It has also been studied for use in bacterial vaginosis and hypospadias post-operative care[1][5][9]. The drug functions primarily through activation of estrogen receptors to exert its local effects on vaginal tissue[2][9].
