Overview

Propafenone is a class 1c antiarrhythmic small molecule drug used to treat rapid heart rhythm disorders such as atrial fibrillation and ventricular arrhythmias. It works primarily by blocking cardiac sodium channels (Sodium channel protein type 5 subunit alpha), thereby slowing the influx of sodium ions into cardiac muscle cells and reducing their excitability. This action prolongs conduction and slightly extends the action potential duration and effective refractory period in cardiac tissue. Propafenone also exhibits beta-adrenergic blocking activity (Beta-1 adrenergic receptor antagonist), which can contribute to its antiarrhythmic effects but may cause bradycardia or bronchospasm in susceptible individuals. The drug is metabolized mainly in the liver and requires dosing two or three times daily due to its short half-life. It is indicated for patients with serious arrhythmias who do not have significant structural heart disease[1][2][8].
