Overview

This entry represents a theoretical combination of four distinct antiarrhythmic drugs: propafenone, flecainide, sotalol, and dofetilide. These agents are used individually or in various combinations to treat and prevent cardiac arrhythmias. Propafenone and flecainide are Class Ic antiarrhythmics, primarily acting as potent sodium channel blockers, which slow cardiac conduction. Propafenone also exhibits weak beta-blocking and calcium channel blocking activity. Sotalol is a Class II (beta-blocker) and Class III antiarrhythmic, blocking both beta-adrenergic receptors and potassium channels (specifically IKr), thereby prolonging cardiac action potential duration. Dofetilide is a pure Class III antiarrhythmic, selectively blocking the rapid component of the delayed rectifier potassium current (IKr), which also prolongs cardiac repolarization. Together, these drugs offer a broad spectrum of antiarrhythmic effects by modulating sodium, potassium, beta-adrenergic, and to a lesser extent, calcium channels in the heart. They are primarily indicated for the management of atrial fibrillation/flutter and ventricular arrhythmias.
