Propamidine is a small molecule aromatic diamidine compound with antiseptic and disinfectant properties. It acts as an antimicrobial agent, exhibiting bacteriostatic activity against a wide range of organisms, including pyrogenic cocci, antibiotic-resistant staphylococci, and some Gram-negative bacilli. Propamidine retains its antibacterial activity in the presence of organic matter such as tissue fluids, pus, and serum. The most commonly used form clinically is the salt propamidine isethionate (formed with isethionic acid), which is primarily indicated for minor eye or eyelid infections such as conjunctivitis and blepharitis, as well as for Acanthamoeba infections (notably keratitis). Its mechanism involves disruption of microbial cell processes; studies suggest it may inhibit melanin biosynthesis in fungi and increase chitin content in fungal cell walls[3][4][8]. It also acts by stopping bacterial growth to allow host immune clearance[7].
