Proparacaine hydrochloride + lidocaine hydrochloride is a combination topical ophthalmic anesthetic regimen used for procedural pain control, specifically during intravitreal injections for retinal diseases. It consists of 0.5% proparacaine hydrochloride drops followed by the application of 4% lidocaine hydrochloride via soaked cotton-tipped swabs held over the injection site. Both agents are small-molecule local anesthetics that function by blocking voltage-gated sodium channels, thereby inhibiting sodium ion permeability across cell membranes and preventing the initiation and conduction of nerve impulses in the cornea, conjunctiva, and sclera. This combination was evaluated in a randomized clinical trial (NCT01027611) led by Illinois Retina Associates to compare its effectiveness against monotherapy and lidocaine gel.
