Prophylactic Donor Lymphocyte Infusion (DLI) is a cellular immunotherapy protocol developed by the Chinese PLA General Hospital to prevent disease relapse in patients with very high-risk hematologic malignancies, including leukemia and lymphoma, following allogeneic peripheral blood stem cell transplantation (allo-PBSCT). The intervention utilizes unmanipulated, G-CSF-primed donor-derived lymphocytes (specifically CD3+ cells) to induce a graft-versus-leukemia (GVL) effect, where donor immune cells recognize and eliminate residual malignant cells in the recipient. For patients harboring specific high-risk mutations such as TP53, TET2, or DNMT3a, the protocol incorporates the administration of decitabine, a hypomethylating agent, prior to the infusion to enhance the GVL effect and potentially sensitize tumor cells to immune-mediated destruction.
