Overview

Propionyl-L-carnitine (PLC) is a propionyl ester of L-carnitine that serves as a critical cofactor in mitochondrial fatty acid metabolism and energy production. By facilitating the transport of long-chain fatty acids into the mitochondria for beta-oxidation, PLC helps restore cellular energy levels in tissues with impaired metabolic function. Additionally, PLC exhibits significant antioxidant properties, acting as a superoxide scavenger and reducing the expression of pro-inflammatory markers such as ICAM-1 and iNOS. In clinical development, PLC is primarily investigated as a colon-release oral formulation for the treatment of mild-to-moderate ulcerative colitis, where it aims to counteract mucosal inflammation and endothelial dysfunction. It has also been extensively studied for its cardiovascular benefits in patients with peripheral arterial disease (intermittent claudication) and chronic ischemia. The drug has been advanced through clinical trials by Sigma-Tau (now Leadiant Biosciences) and Lee's Pharmaceutical.
