Overview

Propofol + esketamine is an intravenous anesthetic combination regimen being investigated for procedural anesthesia and sedation, particularly in gynecological surgeries such as the Loop Electrosurgical Excision Procedure (LEEP). This combination utilizes propofol, a short-acting sedative-hypnotic that functions as a positive allosteric modulator of the GABA-A receptor, and esketamine, the S(+) enantiomer of ketamine that acts as a non-competitive NMDA receptor antagonist. The regimen is designed to provide effective anesthesia with improved hemodynamic stability and reduced side effects compared to single-agent protocols. Clinical research led by Qian Wu and colleagues, published in 2024, evaluated the efficacy and safety of varying esketamine doses in combination with propofol to optimize patient outcomes during LEEP.
