Overview

This is a combination of three intravenous anesthetic agents—propofol, midazolam, and remifentanil—used together for induction and maintenance of general anesthesia or deep sedation. Propofol is a sedative-hypnotic that acts as a positive allosteric modulator at GABA-A receptors, producing rapid onset hypnosis. Midazolam is a benzodiazepine that also enhances GABAergic inhibition via GABA-A receptors, providing anxiolysis, amnesia, and additional sedation. Remifentanil is an ultra-short-acting synthetic opioid agonist at the mu-opioid receptor (OPRM1), providing potent analgesia with rapid onset and offset. The combination allows for synergistic effects in anesthesia induction and maintenance; using all three drugs can reduce the required dose of each agent while achieving effective hypnosis, amnesia, analgesia, and muscle relaxation[5][7]. This regimen is commonly used in surgical settings to optimize patient comfort and safety during procedures.
