Prostatic acid phosphatase-sargramostim fusion protein (PA2024) is the active recombinant fusion protein component of **sipuleucel-T** (Provenge), an autologous cellular immunotherapy for prostate cancer. It consists of human prostatic acid phosphatase (PAP), a prostate-specific tumor antigen, linked to human granulocyte-macrophage colony-stimulating factor (GM-CSF/sargramostim), which serves as an immune-cell activator and adjuvant. In the production of sipuleucel-T, a patient's peripheral blood mononuclear cells (PBMCs), including antigen-presenting cells (APCs), are harvested via leukapheresis and incubated ex vivo with PA2024. The GM-CSF portion directs the fusion protein to APCs and promotes their maturation, while the PAP portion is processed and presented on the APC surface. When reinfused into the patient, these activated APCs stimulate a T-cell mediated immune response against PAP-expressing prostate cancer cells.
