Overview

PROSTVAC + granulocyte-macrophage colony-stimulating factor is a **combination immunotherapy** regimen designed primarily for treating metastatic castration-resistant prostate cancer. **PROSTVAC** is a viral vector-based cancer vaccine utilizing a heterologous prime-boost strategy involving recombinant vaccinia (prime) and fowlpox (boost) vectors, both engineered to express prostate-specific antigen (PSA) and three costimulatory molecules (B7-1, ICAM-1, LFA-3, collectively called TRICOM). Granulocyte-macrophage colony-stimulating factor (GM-CSF; typically sargramostim) is included as an immune adjuvant to potentially enhance the vaccine-induced immune response. The therapy aims to induce robust cytotoxic T-cell responses against prostate cancer cells expressing PSA. Clinical studies have shown the combination is generally well tolerated, mainly resulting in injection site reactions and mild systemic symptoms. While early phase trials suggested improved survival, pivotal Phase III trials found no overall survival benefit compared to placebo[1][2][3][4]. PROSTVAC itself is developed by Bavarian Nordic, and GM-CSF (sargramostim) used in trials is manufactured by Sanofi/Genzyme. The primary clinical indication is metastatic castration-resistant prostate cancer.
