Prothrombin, also known as Coagulation Factor II (FII), is a vitamin K-dependent glycoprotein synthesized in the liver. It is the inactive precursor (zymogen) to thrombin (Factor IIa), a serine protease that plays a central role in the blood coagulation cascade. Upon activation by the prothrombinase complex (composed of Factor Xa, Factor Va, phospholipids, and calcium ions), prothrombin is cleaved to form thrombin. Thrombin then converts soluble fibrinogen into insoluble fibrin, which forms the structural meshwork of a blood clot, and also activates other coagulation factors and platelets. Therapeutically, prothrombin can be administered, often as a component of Prothrombin Complex Concentrates (PCCs), to replenish deficient coagulation factors and restore normal hemostasis, particularly in conditions like prothrombin deficiency or to reverse the effects of certain anticoagulants.
