Overview

The **Protocolized Diuretic Strategy (ProDiuS)** is a standardized clinical intervention protocol developed for the management of fluid overload in patients with **cardiorenal syndrome** (combined heart and kidney failure). Led by **Kelly V. Liang, MD**, at the University of Pittsburgh, the strategy utilizes a 'stepped pharmacologic care' algorithm to overcome diuretic resistance. The protocol involves the aggressive administration of **loop diuretics** (such as **furosemide**) via intravenous bolus followed by continuous infusion, with the potential addition of **thiazide diuretics** (such as oral **metolazone** or intravenous **chlorothiazide**). The primary goal of ProDiuS is to achieve a target urine output of 3-5 liters per day, thereby improving decongestion, reducing hospital length of stay, and enhancing health-related quality of life (HRQOL) compared to non-standardized usual care.
