Overview

PSCA CAR T cells are autologous chimeric antigen receptor T cell therapies genetically engineered to target the prostate stem cell antigen (PSCA), which is commonly and highly expressed in advanced prostate cancer, especially in metastatic castration-resistant prostate cancer (mCRPC). These modified T cells are engineered to express a CAR construct against PSCA, usually including a 4-1BB co-stimulatory domain and CD3ζ signaling, to enhance activation and persistence. The approach seeks to overcome the immunosuppressive tumor microenvironment in prostate cancer by redirecting immune effector cells to tumor cells expressing PSCA. Mechanism of action: The infused CAR T cells recognize and bind PSCA-expressing tumor cells, activating cytotoxic T cell responses to eradicate those cancer cells[1][3][5][7]. Primary indication: mCRPC (metastatic castration-resistant prostate cancer)[1][4][5]. Additional potential indications (under investigation): pancreatic cancer and bladder cancer, where PSCA is also expressed[3][5]. Developers/Manufacturers: Therapy design and production primarily led by research teams at City of Hope, with other investigational products (e.g., BPX-601) by Bellicum Pharmaceuticals for broader PSCA-positive solid tumors[3][5]. Route of administration: intravenous infusion[1][3][4]. Development status: Phase 1 clinical studies are ongoing for mCRPC; phase 1b studies open for enrollment (see clinical trial NCT03873805, NCT05805371)[1][4][6]. Safety: Early phase studies show encouraging signs of efficacy and a favorable toxicity profile, most notably mild cytokine release syndrome and some cystitis; efforts are ongoing to improve T cell persistence and optimize dosing[1][7].
