Pudafensine (IPED2015) is a small molecule drug candidate developed by Initiator Pharma for the treatment of organic erectile dysfunction (ED). It is specifically designed for patients who do not respond adequately to current phosphodiesterase type 5 (PDE5) inhibitors like sildenafil or tadalafil. Pudafensine acts as a triple monoamine reuptake inhibitor (TRI), preferentially binding to and inhibiting the dopamine transporter (DAT), noradrenaline transporter (NAT), and serotonin transporter (SERT). This mechanism increases synaptic levels of monoamines, particularly dopamine in the brain, which initiates the erectile response centrally. Additionally, the drug exerts a direct peripheral effect on erectile tissue by causing the relaxation of erectile smooth muscle. Pudafensine is formulated as an oral tablet and has successfully completed Phase IIb clinical trials, demonstrating statistically significant efficacy in ED endpoints with a favorable safety profile.
