PX-866 + cetuximab is a combination therapy consisting of PX-866, an irreversible, pan-class I phosphatidylinositol 3-kinase (PI3K) inhibitor, and cetuximab, a monoclonal antibody targeting the epidermal growth factor receptor (EGFR). PX-866 inhibits PI3K signaling by blocking the production of phosphatidylinositol-3,4,5-trisphosphate (PIP3), thereby preventing activation of downstream effectors such as AKT and mTOR. This pathway is frequently dysregulated in cancer due to mutations or loss of PTEN. Cetuximab binds to EGFR on tumor cells and blocks ligand-induced activation and downstream signaling involved in cell proliferation and survival. The rationale for combining these agents is based on preclinical evidence that dual inhibition can overcome resistance mechanisms in tumors with aberrant PI3K/AKT/mTOR or EGFR pathways. This combination has been investigated primarily in solid tumors including head and neck squamous cell carcinoma.
