Pyrazinoylguanidine (PZG) is an experimental small molecule derivative of the pyrazinecarboxamide class, structurally related to the diuretic amiloride. Unlike amiloride, PZG does not exhibit significant diuretic or potassium-sparing effects. Instead, it has been investigated for its ability to improve metabolic parameters in patients with non-insulin-dependent diabetes mellitus (NIDDM) and individuals with hydrochlorothiazide-induced metabolic disturbances. PZG works by downregulating the glucose-fatty acid cycle (Randle cycle), which leads to reductions in hyperglycemia, hyperinsulinemia, and hyperlipidemia. Clinical studies have also demonstrated its efficacy in lowering systolic and diastolic blood pressure, even in patients maintained on other antihypertensive medications. The compound was primarily developed and studied in the late 1980s and early 1990s but did not progress to regulatory approval.
