Pyridostigmine is a reversible acetylcholinesterase inhibitor primarily used for the symptomatic treatment of myasthenia gravis and congenital myasthenic syndromes. It works by inhibiting the enzyme acetylcholinesterase at neuromuscular junctions, thereby increasing and prolonging the action of acetylcholine. This enhances communication between nerves and muscles, improving muscle strength in conditions characterized by muscle weakness[1][4][5]. Pyridostigmine does not cross the blood–brain barrier and is also used to reverse non-depolarizing neuromuscular blocking agents after surgery, treat underactive bladder, and as a pretreatment for nerve agent exposure in military settings[5][8]. Off-label uses include immune modulation via stimulation of the cholinergic anti-inflammatory pathway[8].
