Pyrogenal is a highly purified lipopolysaccharide (LPS) preparation derived from the cell walls of *Salmonella typhi*. It functions as a potent non-specific immunomodulator and pyrogenic agent, primarily utilized in Russia and other CIS countries. The drug acts as an agonist for Toll-like receptor 4 (TLR4), triggering the release of endogenous pyrogens such as interleukin-1 (IL-1), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-α). This leads to a controlled febrile response, activation of the reticuloendothelial system, and stimulation of the hypothalamic-pituitary-adrenal axis. Clinically, Pyrogenal is indicated for the treatment of chronic inflammatory conditions, secondary infertility, the resorption of adhesions and scars, and various dermatological disorders including psoriasis and cutaneous mycosis. It is also employed as a provocative agent to detect latent infections and in 'pyrotherapy' to enhance the efficacy of other treatments by stimulating systemic immune defenses.
