Pyrotinib + letrozole is a combination therapy under investigation for the treatment of hormone receptor-positive (HR+), human epidermal growth factor receptor 2-positive (HER2+) metastatic breast cancer. Pyrotinib is an oral, irreversible pan-HER tyrosine kinase inhibitor that targets human epidermal growth factor receptor 1 (EGFR), human epidermal growth factor receptor 2 (HER2), and human epidermal growth factor receptor 4 (HER4) receptors, thereby blocking downstream signaling pathways involved in tumor cell proliferation and survival. Letrozole is a nonsteroidal aromatase inhibitor that suppresses estrogen production by inhibiting the enzyme aromatase (CYP19A1), reducing estrogen-driven tumor growth in postmenopausal women. This combination aims to provide dual blockade of both the human epidermal growth factor receptor 2 (HER2) pathway and estrogen signaling as a chemotherapy-sparing regimen for patients with HR+/HER2+ metastatic breast cancer[1][5][7].
