Pz-1 (and its second-generation variant P28z) is an investigational autologous chimeric antigen receptor (CAR) T-cell therapy developed by Memorial Sloan Kettering Cancer Center for the treatment of metastatic castration-resistant prostate cancer (mCRPC). The therapy involves the genetic modification of a patient's own T cells to express a CAR that targets prostate-specific membrane antigen (PSMA), a cell-surface protein highly expressed on prostate cancer cells and the neovasculature of various solid tumors. The CAR construct utilizes a single-chain variable fragment (scFv) derived from the J591 monoclonal antibody. The Pz-1 construct is a first-generation CAR containing the CD3ζ signaling domain, while the P28z variant incorporates the CD28 costimulatory domain to improve T-cell activation, expansion, and persistence. In clinical trials (NCT01140373), these engineered T cells are administered intravenously following lymphodepleting chemotherapy with cyclophosphamide.
