Qidan Dihuang is a traditional Chinese medicine (TCM) formula used primarily for renal protection, especially in the context of diabetic nephropathy. It is available as both a grain and a decoction formulation. The formula has been studied for its additive effect when combined with angiotensin receptor blockers (ARBs), showing improvement in albuminuria and Traditional Chinese Medicine Symptom scores in patients with type 2 diabetes mellitus who are normotensive[1][2]. Preclinical studies indicate that it can reduce serum creatinine and proteinuria, inhibit renal fibrosis, and modulate the renin-angiotensin system by downregulating proteins such as α-SMA, TGF-β, renin, and AT1[3]. Its mechanism appears to involve inhibition of renal fibrosis pathways and modulation of the RAS system.
