Overview

Qing-Gan-Zi-Shen Decoction (QGZS or QGZST) is a traditional Chinese medicine formula that has been extensively used for decades in China for the treatment of hypertension and metabolic diseases such as obesity, hyperlipidemia, and insulin resistance. It is composed of multiple herbal ingredients (exact composition may vary by source), reflecting the multi-target approach typical of traditional Chinese formulas. Preclinical studies have demonstrated that QGZS exerts cardioprotective effects in animal models of obese hypertension by inhibiting weight gain and reducing blood pressure. Mechanistically, it acts through several pathways: - Upregulation of Nrf2 expression in cardiac tissue (acting as a proximate Nrf2 agonist), which enhances antioxidant defenses and reduces oxidative stress. - Activation of Sirtuin 1 (SIRT1) and Forkhead box protein O1 (FOXO1), leading to improved adipose tissue function. - Downregulation of pro-inflammatory cytokines and suppression of nuclear factor-kappa B-p65 activity. These actions collectively result in reduced cardiac remodeling/hypertrophy, improved systolic/diastolic function, decreased serum cholesterol/triglycerides/glucose/insulin levels, amelioration of adipocyte dysfunction/inflammation, and protection against mitochondrial dysfunction[1][2][5].
