Drug intelligence / Profile preview
Qing-Huang San (QHS) is a traditional Chinese medicine (TCM) herbal formula primarily composed of Realgar (containing tetraarsenic tetrasulfide, $As_4S_4$) and Indigo Naturalis (containing indigo and indirubin). Developed and extensively studied by Xiyuan Hospital of the China Academy of Chinese Medical Sciences, Qing-Huang San is used for the treatment of hematological malignancies, particularly myelodysplastic syndrome (MDS) and acute promyelocytic leukemia (APL). The formula works synergistically: tetraarsenic tetrasulfide acts as the principal active component to induce cell differentiation and apoptosis, while indirubin from Indigo Naturalis acts as an adjuvant to enhance arsenic uptake (via up-regulation of aquaglyceroporin 9, AQP9) and inhibit cyclin-dependent kinases (CDKs). In MDS, Qing-Huang San has been shown to regulate DNA methylation (acting as a dual methyltransferase modulator with both demethylating and pro-methylating effects) and target the TP53/pVHL/HIF1A signaling pathways.
