QL1706 + mFOLFOX6 is a combination therapy being investigated for the treatment of locally advanced rectal cancer (LARC), particularly in patients with proficient mismatch repair/microsatellite stable (pMMR/MSS) tumors. QL1706 (also known as PSB205) is a novel bifunctional MabPair product that co-expresses two engineered monoclonal antibodies targeting Programmed Cell Death Protein 1 (PD-1) and Cytotoxic T-Lymphocyte-Associated Protein 4 (CTLA-4) in a fixed ratio from a single cell line. mFOLFOX6 is a standard chemotherapy regimen consisting of oxaliplatin, leucovorin, and 5-fluorouracil. In clinical trials led by Sun Yat-sen University, this combination is administered as part of a total neoadjuvant therapy (TNT) approach, often following short-course radiotherapy (SCRT). The strategy aims to leverage the synergistic effects of radiation and chemotherapy to prime the immune microenvironment, potentially overcoming the typical resistance of pMMR/MSS rectal cancers to single-agent immune checkpoint inhibition.
