Overview

R(+)-propranolol, also known as dexpropranolol, is the dextrorotatory enantiomer of the non-selective beta-adrenergic receptor antagonist propranolol. While the S(-)-enantiomer is primarily responsible for the potent beta-blocking activity used in cardiovascular medicine, R(+)-propranolol exhibits significantly lower affinity for beta-adrenoceptors. Historically, it was developed by AstraZeneca (formerly ICI Pharmaceuticals) and has been used primarily as a research tool to isolate the non-adrenergic effects of the racemic mixture. Recent preclinical research has highlighted its potential in oncology, specifically for colorectal cancer, where it has been shown to inhibit the MAPK signaling pathway and downregulate the expression of PD-1 on T cells, thereby suppressing tumor growth in a time- and concentration-dependent manner.
