R-bambuterol (specifically R-bambuterol hydrochloride) is the (R)-enantiomer of the racemic drug bambuterol, a carbamate prodrug of the selective beta-2 adrenergic receptor agonist terbutaline. Developed by Dongguan Kaifa Biopharmaceutical Co., Ltd., R-bambuterol is currently in Phase II clinical development for the treatment of mild-to-moderate inflammatory bowel disease (IBD), including ulcerative colitis and Crohn's disease. While racemic bambuterol is traditionally used as a long-acting bronchodilator for respiratory conditions like asthma, the R-enantiomer is being explored for its immunomodulatory and anti-inflammatory properties in the gastrointestinal tract, where beta-2 adrenergic signaling is thought to play a role in regulating mucosal immunity.
