Overview

R-CHOP + blinatumomab is an investigational chemo-immunotherapy regimen being evaluated by the French Innovative Leukemia Organisation (FILO) for the treatment of Richter Syndrome (RS), specifically the diffuse large B-cell lymphoma (DLBCL) histological subtype. The regimen consists of an initial debulking phase using R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) to reduce high tumor burden, followed by induction therapy with blinatumomab. Blinatumomab is a bispecific T-cell engaging (BiTE) antibody that simultaneously binds to CD19 on B-cells and CD3 on T-cells, facilitating the directed lysis of malignant B-cells by the patient's own T-cells. This sequential approach is being studied in the Phase 2 BLINART trial to determine if it can improve complete response rates in patients with Richter transformation, a condition that typically responds poorly to conventional chemotherapy.
