Overview

R-etodolac (SDX-101) is the R-enantiomer of the nonsteroidal anti-inflammatory drug (NSAID) etodolac. Unlike the S-enantiomer, which is responsible for cyclooxygenase (COX) inhibition and associated anti-inflammatory effects, R-etodolac is a non-COX-inhibiting compound that exhibits potent pro-apoptotic and antitumor activity. Its mechanism of action involves binding to the Retinoid X Receptor (RXR) and downregulating the Wnt/beta-catenin signaling pathway, as well as modulating PPAR-gamma activity. This leads to the selective induction of apoptosis in malignant cells, particularly in B-cell chronic lymphocytic leukemia (B-CLL) and multiple myeloma. Developed by Salmedix (later acquired by Cephalon), R-etodolac was designed to provide a safer therapeutic profile by avoiding the gastrointestinal and renal toxicities typically associated with traditional NSAIDs.
