R-HDS (IRCCS Ospedale San Raffaele)

R-HDS is an intensive, multi-phase chemoimmunotherapy regimen developed by Dr. Andrés José María Ferreri for the treatment of aggressive B-cell lymphomas with secondary central nervous system (CNS) involvement. The protocol consists of a sequential administration of high-dose chemotherapy agents designed to maximize blood-brain barrier penetration and overcome drug resistance. It typically includes an induction phase with rituximab, high-dose methotrexate, and cytarabine, followed by an intensification phase (R-HDS) using high-dose cyclophosphamide, cytarabine, and etoposide. CNS prophylaxis is provided via intrathecal liposomal cytarabine. The regimen is consolidated with high-dose thiotepa and carmustine (BCNU) conditioning followed by autologous stem cell transplantation (ASCT) to restore hematopoiesis.
