Overview

R-miniCHOP + acalabrutinib is a combination therapy used in the treatment of diffuse large B-cell lymphoma (DLBCL), particularly in older or frail adults. R-miniCHOP is an attenuated version of the standard CHOP chemotherapy regimen and includes rituximab (a monoclonal antibody targeting CD20), cyclophosphamide, doxorubicin, vincristine, and prednisolone. Acalabrutinib is a highly selective, covalent small molecule inhibitor of Bruton tyrosine kinase (BTK), which plays a critical role in B-cell receptor signaling. The addition of acalabrutinib to R-miniCHOP aims to improve outcomes by further inhibiting malignant B-cell proliferation and survival. Clinical trials have shown that this combination results in increased but manageable toxicity without an increased rate of treatment-emergent deaths[1][3][8]. The regimen is under investigation for its potential to prolong progression-free survival in untreated DLBCL patients who are not candidates for full-dose chemotherapy.
