Overview

R-OKY-034F is an orally bioavailable small molecule inhibitor of monocarboxylate transporter 4 (MCT4), currently under clinical investigation for the treatment of refractory or intolerant, surgically unresectable pancreatic ductal adenocarcinoma (PDAC). Developed as a spin-off from research at Osaka University and advanced by the venture company Okyanos, the drug targets the metabolic vulnerabilities of pancreatic cancer cells. MCT4 is a transmembrane protein responsible for the efflux of lactate, a byproduct of glycolysis; in the highly glycolytic and hypoxic microenvironment of pancreatic tumors, MCT4 is often upregulated to prevent intracellular acidification. By inhibiting MCT4, R-OKY-034F promotes the accumulation of intracellular lactate and protons, leading to metabolic stress, intracellular acidification, and subsequent apoptosis in tumor cells. It is being evaluated in Phase I/IIa clinical trials in Japan, supported by the Japan Agency for Medical Research and Development (AMED).
