Rabeprazole is a small molecule pharmaceutical drug in the class of proton pump inhibitors (PPIs). It is primarily used to treat conditions involving excessive stomach acid production, such as gastroesophageal reflux disease (GERD), peptic ulcer disease, and Zollinger-Ellison syndrome. Rabeprazole is also used in combination with antibiotics to eradicate Helicobacter pylori infection associated with duodenal ulcers. The drug acts by irreversibly inhibiting the gastric H+, K+-ATPase enzyme (the "proton pump") located on the secretory surface of gastric parietal cells. This inhibition blocks the final step of gastric acid secretion, leading to reduced acidity in the stomach and allowing healing of acid-related damage[1][2][4][5][6]. Rabeprazole is a prodrug that becomes activated in acidic environments within parietal cells and forms covalent bonds with cysteine residues on the proton pump[5]. It has a relatively rapid onset compared to other PPIs due to its higher pKa value[8].
