This combination therapy, investigated by the National Taiwan University Hospital, is a regimen designed for the first-line eradication of *Helicobacter pylori* infection. It consists of three primary components: rabeprazole, amoxicillin, and tripotassium dicitrate bismuthate. Rabeprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion by inhibiting the H+/K+-ATPase enzyme, thereby creating a higher pH environment that optimizes the stability and antimicrobial activity of amoxicillin. Amoxicillin is a beta-lactam antibiotic that targets penicillin-binding proteins to inhibit bacterial cell wall synthesis, leading to bacterial lysis. Tripotassium dicitrate bismuthate is a bismuth salt that provides multi-modal support through direct bactericidal effects on *H. pylori*, inhibition of bacterial enzymes like urease, and prevention of bacterial adhesion to the gastric mucosa. This specific high-dose dual therapy plus bismuth (B-HDDT) protocol aims to improve eradication rates and patient compliance compared to traditional quadruple therapies.
