Radioiodinated anti-carcinoembryonic antigen M5A is a radiopharmaceutical comprising the humanized monoclonal antibody M5A labeled with a radioisotope of iodine, such as Iodine-131 (131I) or Iodine-124 (124I). M5A is an intact IgG1 antibody engineered from the murine T84.66 antibody to target carcinoembryonic antigen (CEA; CEACAM5), a glycoprotein frequently overexpressed in solid tumors, particularly colorectal and pancreatic carcinomas. The radioiodinated conjugate is designed for targeted imaging (immuno-PET) or radioimmunotherapy, leveraging the high specificity of the M5A antibody for CEA-expressing cells to deliver localized radiation. Preclinical studies have demonstrated that M5A exhibits superior tumor retention and a longer serum half-life compared to smaller antibody fragments like diabodies or minibodies, although it is subject to in vivo dehalogenation (loss of the iodine label).
