Radiolabeled BI 1291583 is a version of the small molecule BI 1291583, an oral, highly potent, and selective inhibitor of Cathepsin C (also known as Dipeptidyl peptidase 1, DPP1), developed by Boehringer Ingelheim. Cathepsin C is a lysosomal cysteine protease responsible for the activation of pro-inflammatory neutrophil serine proteases (NSPs), including neutrophil elastase, proteinase 3, and cathepsin G, during myelopoiesis in the bone marrow. By inhibiting Cathepsin C, BI 1291583 reduces the activity of these NSPs, which are key drivers of airway inflammation and tissue damage in chronic inflammatory lung diseases such as non-cystic fibrosis bronchiectasis (NCFB). The radiolabeled form, typically incorporating Carbon-14 ([14C]), is specifically utilized in Phase 1 clinical pharmacology studies to characterize the drug's absorption, distribution, metabolism, and excretion (ADME) profile, providing essential data on its pharmacokinetic properties and metabolic pathways in humans.
