Radium‑223 dichloride is a radiopharmaceutical agent used primarily to treat men with castration-resistant prostate cancer (CRPC) that has metastasized to the bones but not to other organs. It is an alpha particle-emitting therapeutic radiopharmaceutical. Radium‑223 mimics calcium and selectively targets areas of increased bone turnover, such as sites of bone metastases, by binding to hydroxyapatite in the bone matrix. Once localized, it emits high-energy alpha particles over a very short range (<100 micrometers), causing double-strand DNA breaks in adjacent cells—including tumor cells—resulting in cell death and an anti-tumor effect on bone metastases while minimizing damage to surrounding healthy tissue. The drug was first approved by the FDA in May 2013 and is marketed under the brand name Xofigo[1][2][4][6][7].
