**Radium-223-doped calcium-alginate microspheres** are a preclinical alpha-particle transarterial radioembolization platform for **hepatocellular carcinoma**. The product incorporates the alpha-emitting radionuclide radium-223 into a calcium-alginate hydrogel microsphere matrix through in-situ doping; alginate egg-box coordination retains radium ions within the microspheres after selective hepatic arterial delivery. Localized alpha-particle emission delivers high-linear-energy-transfer radiation to tumor tissue, causing cytotoxic DNA damage under hypoxic conditions and promoting immunogenic cell-death and antigen-presentation pathways. In orthotopic rat hepatocellular carcinoma studies, the microspheres showed intratumoral retention, antitumor activity, dendritic-cell maturation, and increased CD8-positive T-cell infiltration.
