RAG-01 is a pioneering small activating RNA (saRNA) therapy developed by Ractigen Therapeutics for the treatment of non-muscle invasive bladder cancer (NMIBC), particularly in patients unresponsive to Bacillus Calmette–Guérin (BCG) therapy. It is designed to upregulate the p21 tumor suppressor gene, a key regulator of cell cycle progression that has been challenging to target with traditional therapies. The drug works via RNA activation (RNAa), using saRNA molecules delivered directly into the bladder through intravesical instillation and employing proprietary LiCO delivery technology for targeted urothelial uptake and minimal systemic exposure. Preclinical studies have shown significant tumor suppression, and early clinical data from an ongoing Phase I trial demonstrate promising complete response rates in carcinoma in situ and papillary tumors, as well as a favorable safety profile with no dose-limiting toxicities observed. RAG-01 received FDA Fast Track designation in May 2024, marking it as the first saRNA drug worldwide to achieve this status for NMIBC.
