Ralinepag is an orally administered, potent, and selective small molecule agonist of the prostacyclin (IP) receptor developed for the treatment of pulmonary arterial hypertension (PAH). It is a non-prostanoid compound designed to mimic the effects of endogenous prostacyclin by activating IP receptors, leading to vasodilation, inhibition of smooth muscle cell proliferation, and reduced platelet aggregation. Ralinepag has demonstrated dose-dependent efficacy in preclinical models and significant reduction in pulmonary vascular resistance in clinical trials. Its extended-release formulation allows for once-daily dosing with a prolonged half-life and lower peak-to-trough fluctuations compared to immediate-release forms. The drug was originally developed by Arena Pharmaceuticals and is currently being further developed by United Therapeutics Corporation[3][4][5][6][7].
