The combination of the **Ralox regimen** (raltitrexed and oxaliplatin) with **anlotinib** and **penpulimab** is an investigational four-drug chemo-immunotherapy regimen being evaluated for the treatment of unresectable hepatocellular carcinoma (HCC). The regimen combines locoregional/systemic chemotherapy (the Ralox backbone) with anlotinib, a small-molecule multi-target tyrosine kinase inhibitor (TKI) targeting VEGFR, FGFR, PDGFR, and c-Kit, and penpulimab, a humanized anti-PD-1 IgG1 monoclonal antibody. This combination therapy aims to leverage synergistic anti-tumor mechanisms, including DNA damage, thymidylate synthase inhibition, anti-angiogenesis, and immune checkpoint blockade, to improve outcomes in patients with advanced liver cancer. The regimen is currently being studied in a prospective clinical trial led by Nanfang Hospital, Southern Medical University.
