This combination therapy consists of the angiotensin-converting enzyme (ACE) inhibitor ramipril and the P2Y12 receptor antagonist clopidogrel. It was investigated by AHEPA University Hospital in a Phase 4 clinical trial (NCT00411112) to evaluate its effects on endothelial function, oxidative stress, and vascular inflammation in patients with type 2 diabetes and diabetic nephropathy. Ramipril functions by inhibiting the conversion of angiotensin I to angiotensin II, thereby reducing vasoconstriction and aldosterone secretion, which provides renal and cardiovascular protection. Clopidogrel is an antiplatelet agent that irreversibly inhibits the P2Y12 subtype of ADP receptors on platelets, preventing activation of the GPIIb/IIIa receptor complex and subsequent platelet aggregation. The study aimed to determine if the addition of clopidogrel to standard ramipril therapy offers synergistic benefits in reducing vascular complications associated with diabetic kidney disease.
