Overview

Ranolazine is an antianginal small molecule medication used for the treatment of chronic angina (chest pain) in adults. It is typically prescribed when other antianginal drugs are insufficient or as an add-on therapy with agents such as beta-blockers, calcium channel blockers, nitrates, ACE inhibitors, or lipid-lowering medications[1][2][4]. The precise mechanism of action is not fully understood; however, ranolazine inhibits the cardiac late sodium current (INa), which helps reduce intracellular sodium and calcium overload in ischemic cardiac myocytes. This effect improves myocardial relaxation and reduces angina symptoms without significantly affecting heart rate or blood pressure[4]. Ranolazine also blocks the rapid component of the delayed rectifier potassium current (IKr), prolonging the QTc interval in a dose-dependent manner[4]. It was originally approved by the FDA in 2006.
