RAP-219 is a clinical-stage small molecule drug developed by Rapport Therapeutics. It acts as a highly potent and selective negative allosteric modulator of the AMPA receptor through its specific targeting of the AMPA-associated protein TARPγ8. This mechanism is designed to achieve neuroanatomical specificity, as TARPγ8 expression is enriched in the hippocampus and cerebral cortex but minimal in the cerebellum and brain stem, potentially reducing side effects commonly seen with less selective anti-seizure medications. RAP-219 has demonstrated favorable safety and tolerability profiles in Phase 1 trials, with no sedation or motoric impairments observed. The drug has a long half-life (8–14 days) and minimal drug-drug interactions, making it suitable for polypharmacy settings. Currently, RAP-219 is being evaluated for focal epilepsy (with an ongoing Phase 2a trial), diabetic peripheral neuropathic pain, and bipolar mania[1][2][3][4][5][6][8].
