RAP-NGF is a recombinant fusion protein designed to deliver Nerve Growth Factor (NGF) across the blood-brain barrier (BBB) for the treatment of neurodegenerative disorders, most notably Alzheimer's disease. The molecule consists of the 39 kDa receptor-associated protein (RAP), which serves as a high-affinity ligand for the low-density lipoprotein receptor-related protein 1 (LRP1), fused to the neurotrophic factor NGF. LRP1 is highly expressed on the surface of brain capillary endothelial cells and facilitates the transport of ligands into the brain parenchyma via receptor-mediated transcytosis. By leveraging this pathway, RAP-NGF enables the systemic administration of NGF, which otherwise cannot cross the BBB, to reach target neurons in the central nervous system. Once across the BBB, the NGF moiety binds to its high-affinity receptor, TrkA, and the low-affinity receptor, p75NTR, to promote neuronal survival, neurite outgrowth, and cholinergic function.
