RAPA-201 is a novel, autologous, polyclonal T-cell therapy developed by Rapa Therapeutics. Unlike genetically engineered CAR-T cells, RAPA-201 consists of T cells that are epigenetically reprogrammed and metabolically engineered ex vivo. The manufacturing process involves stringent mTOR inhibition using temsirolimus and polarization with IFN-α to create a rapamycin-resistant, Th1/Tc1-polarized product. This process enriches the T central memory (TCM) subset, reduces markers of senescence (CD27, KLRG), and downregulates multiple immune checkpoints (PD-1, CD73, LAIR1). RAPA-201 is designed to provide a safer alternative to CAR-T therapies, avoiding toxicities like cytokine release syndrome (CRS) and neurotoxicity (ICANS), while maintaining efficacy in heavily pretreated patients with relapsed/refractory multiple myeloma and various solid tumors.
