Rapamycin (sirolimus) is a macrolide small molecule that functions as a potent and selective inhibitor of the mechanistic target of rapamycin complex 1 (mTORC1). Originally developed as an antifungal agent and later FDA-approved as an immunosuppressant for organ transplantation, rapamycin is being repurposed by AgelessRx for its potential to slow the biological aging process and extend healthspan. The PEARL trial (Participatory Evaluation of Aging with Rapamycin for Longevity) evaluated the safety and efficacy of low-dose, intermittent weekly administration (5 mg or 10 mg) in healthy older adults. By modulating the mTOR pathway, rapamycin mimics the effects of caloric restriction, promotes autophagy, and reduces cellular senescence, which may lead to improvements in lean tissue mass and reductions in age-related inflammation.
