RBEX is a proprietary, human heart-derived exosome biotherapeutic being developed by Rhythm Biotherapeutics (a spinout from the University of Ottawa Heart Institute) for the prevention and treatment of atrial fibrillation, with a primary focus on post-operative atrial fibrillation (POAF). RBEX consists of naturally occurring extracellular vesicles carrying native microRNAs (such as miR-22-3p, miR-199b-5p, and miR-374a-5p) and proteins (such as surface decorin) that act synergistically to suppress inflammation and fibrosis. Mechanistically, RBEX targets the NLRP3 inflammasome pathway, reducing caspase-1 activation and the subsequent release of IL-1β and IL-18, while surface decorin inhibits TGF-β-driven fibroblast proliferation. This dual action limits electrical and structural remodeling of the atrium, preventing the onset of atrial fibrillation.
